We will be closing May 29, 2009
Due to the demands of The FACE Clinic and The FACE and Body Clinic of Hawaii, we will be closing Alabama ENT Head & Neck Surgery Center effective May 29, 2009. We would like to thank all of our patient who have trusted us to assist with their medical care. Fortunately, there are several great Otolaryngologists in the area including those at EntCare, and we encourage you contact them for any further ENT concerns.
As required by law, we will be retaining all records for the next several years. We will be more than happy to forward your medical records with the proper release of information form. These are available at our office, via mail, fax, or can be downloaded here.
